The Effect of Individually Tailored Group Aerobic Exercise in Patients with Chronic Mental Illness

Abstract

|
Objectives : Chronic mental illness such as schizophrenia is characterized by lower participation in physical activity, which is associated with negative symptoms, depression and poorer functional outcome, compared to the general population. The objective of this study was to examine the feasibility and effect of physical, mental health of an individually tailored group outdoor exercise program in patients with chronic mental illness.
Methods : Twenty-five patients with chronic mental illness participated in one group-based individually tailored 90-minute outdoor cycling session per week for 3 months. Physical health was evaluated by obesity-related anthropometric measures, step test, and blood tests. Mental health was assessed using the Personal Behavioral Analysis (PBA), Rosenberg Self-Esteem Scale (RSE), Relationship Change Scale (RCS), Quality of Life Scale (WHOQOL-BREF), Global Assessment of Functioning (GAF), and Clinical Global Impression (CGI).
Results : Twenty-three patients completed 3-month aerobic exercise program. Exercise significantly increased participant selfesteem (p=0.016), positive relationship (p=0.002), global function (p=0.001), clinical impression (p=0.001). Exercise tended to improve overall Quality of Life (p=0.053) and significantly improved its sub-domain : physical health (p=0.001). Stress coping methods such as adaptive behavior (p=0.001) and short-term coping method (p=0.022) were significantly changed after exercise program. Cardiorespiratory (CR) fitness significantly improved (p=0.001) after 3 months.
Conclusion : Individually tailored group outdoor exercise program led to significant improvements in functional mental health outcomes and CR functions. Considering physical and mental health improvements in this study despite limited exercise intensity and frequency, individually tailored group outdoor cycling exercise has high probability of important therapeutic technique in patients with chronic mental illness.
